What it is

Some lines appear only when you move — the horizontal lines when you raise your brows, the vertical ones when you frown, the fan at the outer eye when you smile. Those are dynamic lines, caused by muscle contraction folding the skin over and over. Botulinum toxin reduces the contraction, so the fold stops forming and the line softens.

Lines that are visible when your face is completely still are a different problem. They are etched into the skin, and they need collagen treatment or filler, not toxin. Part of the consultation is separating the two, because treating a static line with toxin produces a disappointed patient.

Our approach is deliberately conservative. Under-dosing can be topped up at your two-week review; over-dosing cannot be undone and you live with it for three months. A heavy, frozen forehead and a drooping brow both come from too much product placed without regard for how strong the individual muscle is. Starting lower and reviewing is simply the safer order.

Who it suits — and who it doesn't

This list is not exhaustive. Your medical history, medications and previous treatments are reviewed at consultation, and that review sometimes changes the plan.

The procedure, step by step

1. Assessment. We watch your face move, test the strength of each muscle group, note any existing brow or lid asymmetry, and photograph at rest and in expression.
2. Planning. Injection points and units are decided for your anatomy. Strong muscles need more, and one side is often stronger than the other.
3. Cleaning. The skin is cleaned with antiseptic. Numbing is not usually needed; ice can be used if you prefer.
4. Injection. Several tiny injections with a very fine needle. Each feels like a brief pinprick. The whole thing takes a few minutes.
5. Aftercare instructions. Stay upright for four hours, do not rub or massage the area, no gym, sauna or facial that day.
6. Two-week review. We see you again to check symmetry and effect, and top up if a muscle needs a little more. This review is part of the treatment, not an extra.

Recovery and aftercare

There is no downtime. You may have small red bumps at the injection points for 20 minutes and occasionally a small bruise that lasts a few days.

For four hours, stay upright and do not lie flat, rub the area or wear anything pressing on your forehead. Skip the gym, steam room and facials for 24 hours. Avoid anti-inflammatory painkillers and alcohol on the day, as both increase bruising.

Mild headache in the first day or two is not unusual and settles.

Results you can expect

You will notice softening from day three, with the full effect at ten to fourteen days. That is when the result is judged and adjusted — not at day three.

It lasts three to four months on average. The first treatment sometimes wears off a little sooner; with regular treatment the interval often lengthens, because the muscle is being used less. When it wears off, your movement returns exactly as before — nothing is worse for having had it.

Deeper etched lines improve but do not disappear from toxin alone. Where a line has been folded into the skin for years, expect softening rather than erasure.
